Position: Ryan White HIV/AIDS Case Manager
Location: Mecklenburg County, NC (In-Office, Community-Based, and Remote/Phone)
Reports To: Lillian Johnson, Case Management Supervisor
Position Overview
We are seeking a dedicated, compassionate, and highly organized Ryan White Case Manager to join our team in Mecklenburg County. This role is vital in supporting individuals living with HIV/AIDS by removing barriers to care and improving health outcomes.
The successful candidate will be a proactive advocate, comfortable working in a hybrid capacity that includes office-based administration, telephonic outreach, and direct engagement within the community (including home visits and medical escorting).
Key Responsibilities
Comprehensive Case Management: Conduct initial intakes, psychosocial assessments, and annual eligibility recertifications for Ryan White Program services.
Care Coordination: Develop and implement Individualized Service Plans (ISPs) focused on viral suppression and treatment adherence.
Advocacy & Linkage: Connect clients to primary medical care, mental health support, housing assistance, and pharmacy programs.
Community Outreach: Meet clients where they are. This role requires traveling within Mecklenburg County to conduct home visits and community-based assessments.
Documentation: Maintain precise, HIPAA-compliant records and data entry within specialized tracking systems (e.g., CAREWare).
Qualifications & Requirements
Education: Bachelor’s Degree in Social Work, Human Services, Public Health, or a related field (Master’s degree or BSW preferred).
Experience: Minimum of 1–2 years of experience in case management, preferably working with HIV/AIDS or other chronic health conditions.
Work Environment: Must be willing and able to work in the office, conduct business via phone, and travel throughout the community to meet with clients.
Communication: Excellent interpersonal skills and the ability to work with diverse populations with empathy and cultural humility.

Contract Registered Nurse (RN) – Personal Care Services
Location: Charlotte, NC
Position Type: Independent Contractor (1099) / Professional Services Agreement
Program: North Carolina Medicaid Personal Care Services (PCS)
Position Overview
We are seeking a highly organized and clinical North Carolina Registered Nurse (RN) to support our Personal Care Services (PCS) program on a contract basis. The successful candidate will ensure clinical compliance, oversee the quality of care provided by in-home aides, and maintain audit-readiness in accordance with NC Medicaid policies and NC Home Care Licensure Rules.
Key Responsibilities
Clinical Oversight: Write, review, and update comprehensive Plans of Care (POC) for clients.
Supervision: Supervise PCS aides to ensure service delivery aligns strictly with established plans of care.
Field Visits: Conduct and document 90-day supervisory visits in client homes to assess service quality and client health status.
Competency Validation: Evaluate and document aide competency, with a specific focus on Activities of Daily Living (ADL) assistance and specialized tasks requiring higher-level aide skills.
Compliance & Quality Assurance: Oversee quality management, maintain accurate complaint logs, and ensure all service records meet NC Medicaid compliance standards.
Audit Readiness: Support the agency in staying survey-ready and Medicaid-compliant through meticulous documentation and policy adherence.
Minimum Requirements
Licensure: Active, unrestricted North Carolina Registered Nurse (RN) license.
Experience: Proven experience with NC Medicaid PCS, home care, or in-home aide supervision.
Regulatory Knowledge: Deep familiarity with NC Home Care Licensure Rules, NC Medicaid PCS policy, and NC Board of Nursing requirements.
Documentation Skills: Exceptional clinical documentation skills with a focus on Medicaid compliance and audit-readiness.
Employment Status: Comfortable working as an independent contractor (1099) or under a professional services agreement.
Preferred Qualifications
Prior experience working with behavioral health populations, supportive housing, HIV/AIDS care, or medically complex adults.
Experience in policy development and staff training.
Background in corrective action planning and compliance support for healthcare agencies.
